Local physician governance and continued practice ownership preserve clinical autonomy, while centralized medical groups, management-services organizations, and accountable care organizations coordinate reimbursement and population health.

Some medical groups are owned or consolidated; others are served under management agreements or state-specific friendly-physician structures. The model depends on provider affiliations, payer and health-system relationships, government reimbursement programs, healthcare and privacy rules, and an athenahealth-based electronic medical record.
