Owns health-care real estate, primarily skilled-nursing facilities, and earns rent under long-term triple-net leases while tenants handle operations and property costs. Relationship-led sourcing and active tenant monitoring support acquisitions, while tenant reimbursement and related-party lease exposure shape the model.
Owns skilled-nursing facilities, assisted-living facilities, and long-term acute-care hospitals across a multi-state U.S. footprint, leasing them mainly to third-party operators through long-term triple-net arrangements. The model generates rent while tenants bear property taxes, insurance, maintenance, repairs, operating costs, and capital expenditures; skilled nursing supplies nearly all of the rent base.
Sources health-care properties through operator relationships, including off-market and smaller transactions, and monitors tenants with monthly financial information and facility visits. Its portfolio and rent base depend on tenants' ability to operate under Medicare, Medicaid, and other reimbursement systems, while master leases connect obligations across facilities. Affiliates of the chief executive and a director account for a substantial portion of leased facilities and rent, and those related-party leases are not negotiated at arm's length.
