More than 300 facilities span the United States, and skilled nursing generates nearly all patient and resident service revenue.

Expands mainly by acquiring facilities, investing in clinical teams, equipment, and physical plants, and using PACS Services for technology and back-office support. Local administrators retain day-to-day operating authority while regional and central teams support integration, compliance, staffing, billing, and risk management.

Revenue is concentrated in Medicare and Medicaid reimbursement, while results depend on clinical staffing, hospital referrals, care quality, licensing, and program-participation compliance. Ongoing government investigations and material weaknesses in financial controls remain unresolved; most facilities operate under leases.
